INTUBACIJA U BRZOM SLEDU

Sažetak


U urgentnoj medicini ne postoji postupak koji je važniji od zbrinjavanja disajnog puta. U literaturi su predložene brojne metode koje se koriste kod kompromitovanog disajnog puta a jedna od njih je intubacija u brzom sledu (rapid sequence intubation - RSI). Prema definiciji, RSI je koordinisani, sekvencijalni proces pripreme, sedacije i paralize pacijenta sa visokim rizikom od aspiracije želudačnog sadrža, kako bi se omogućila hitna endotrahealna intubacija (ETI). Farmakološka sedacija i paraliza se indukuju u brzom sledu kako bi se obezbedili optimalni uslovi za laringoskopiju i ETI. RSI se kao napredna tehnika zbrinjavanja disajnog puta koristi kod kritično obolelih i teško povređenih pacijenata u cilju zaštite disajnih puteva od regurgitacije i aspiracije želudačnog sadržaja. Primenu tehnike RSI, sastavljene od 15 koraka, su prvi opisali Stept i Safar 1970. Danas ona predstavlja zlatni standard za urgentno zbrinjavanje disajnog puta kod brzog uvođenja u opštu anesteziju. U novije vreme, RSI se primenjuje i na prehospitalnom nivou kao alternativna metoda za zbrinjavanje disajnog puta pacijenata sa poremećajem svesti, kod kojih se pretpostavlja da imaju pun stomak a postoji šansa za njihovim preživljavanjem.
